Insurance InfoINSURANCE AND HEALTH PLANS: Spectrum Rehabilitation participates in nearly all
insurance companies' health plans and workers' compensation plans, with new
panels being added monthly. We are also participating Medicare Providers.
With all insurance, you are responsible for meeting any deductibles and
co-insurances as indicated by your plan. Please bring your insurance/health plan card and referral or prescription from your doctor to your appointment. Please see the list of insurance companies that we are currently participating in.
Spectrum Rehabilitation is pleased to offer on-site insurance benefit and verification. With information from your insurance card and some personal data, our trained staff will obtain insurance benefits and notify you of your responsibilities and coverage prior to your appointment at your request. All financial responsibilities can be discussed at this time.
If you have an insurance that we do participate or contract with, we will submit your insurance claims for you pending all information is provided. Required information includes: a copy of your insurance/health plan card, an address and phone number to submit claims and verify your coverage. Co-payments are expected at the time of your appointment. If you are experiencing financial difficulties, payment plans can be established. In the event your coverage is denied for any reason, you are responsible for payment of your balance based on our normal fee schedule.
OUT-OF-NETWORK/ OR NO INSURANCE: If you have an insurance that we do not contract with, you may choose to pay at an out-of-network rate, or choose to participate in a cash pay plan. Out-of-network benefits will be discussed with you prior to your appointment, and should you choose to continue with your insurance, you will be responsible for any deductibles and coinsurance or co-payments that you have. For Cash Pay patients, a discounted rate is available within Medicare guidelines, and payment is expected at the time of service. Cash, check or credit card payments are accepted.
WORKERS' COMPENSATION/AUTO INSURANCE: In order to obtain benefit verification and submit claims for work related injuries and auto injuries, information regarding the date of injury or accident, where the accident occurred and the claim number. In addition, the name and phone number of your adjuster for your case is required.
